About Us We are a compassionate, patient-centered healthcare team dedicated to providing high-quality palliative care services to individuals with serious illness. Our mission is to enhance quality of life through symptom management, emotional support, and holistic, individualized care for patients and their families. Position Summary We are seeking a skilled and compassionate Palliative Care Nurse Practitioner (NP) to join our team. The ideal candidate will provide comprehensive, interdisciplinary care focused on symptom management, goals-of-care discussions, and support for patients facing serious or life-limiting illnesses. This role offers the opportunity to build meaningful relationships with patients and families while working in a supportive, collaborative environment. Key Responsibilities Conduct comprehensive patient assessments, including physical, psychosocial, and spiritual needs Develop and implement individualized care plans focused on comfort and quality of life Manage complex symptoms (pain, dyspnea, nausea, anxiety, etc.) Facilitate goals-of-care conversations and advance care planning Collaborate with physicians, nurses, social workers, and interdisciplinary team members Provide education and support to patients and families Coordinate care across settings (home, clinic, hospital, or hospice as applicable) Maintain accurate and timely documentation Qualifications Current Nurse Practitioner license in Colorado Board certification as a Family, Adult-Gerontology, or Acute Care NP Minimum of 12 years of clinical experience (palliative care, hospice, geriatrics, or primary care preferred) Strong communication and interpersonal skills Ability to work independently and as part of a team DEA license (or ability to obtain) Preferred Qualifications Experience in palliative care or hospice Certification in Hospice and Palliative Nursing (CHPN or ACHPN) Experience with serious illness conversations and end-of-life care What We Offer Competitive salary and benefits package Flexible scheduling options Supportive, mission-driven work environment Opportunities for professional growth and continuing education Meaningful, relationship-based patient care Why Join Us?
