Longmont Family Medicine is looking for a NP or PA to join us! If you are a lifelong learner and interested to join us, we are waiting to welcome you. Overview of Practicing Medicine with Us We are intentional to prioritize patient-provider relationships and the people in those working relationships that improve health and wellbeing. This difference is possible in a privately owned and operated practice where your input and ideas matter in growing a better place together alongside a clinical pharmacist, certified health coach, behavioral health clinicians, and care management. AthenaOne electronic medical records, Provider Lounges, Retreats, and helping individuals and families that respect and fulfill why you are in medicine. A Provider is hands-on and delivers full-scope health care using evidence-based guidelines and practice-initiatives within the clinic. The Provider evaluates and manages patient care, to include patient wishes, preferences, and goals in acute, chronic, and preventive & well-care for children, adolescents, and adults. Education with patients and/or families as to the nature of disease, provides instruction in evidence-based options, and discusses risk-benefits of care are all important to patients. The Provider performs medical procedures based on knowledge, skills, and interests. We ask all Providers to demonstrate knowledge of the practice protocols, including comfort in immunizations, well-child and adult care, care management, comprehensive care planning, and health-needs screening, cognitive assessment, and frailty assessment and more. Compensation at hire is competitive and based on credentials and experience.
- 4-day workweek (32-40 direct patient care hours) - Mondays, Tuesdays, Wednesdays, Fridays (Thursdays off) Education & Experience -
- Minimum of at least 5-10 years of experience in Family Medicine, preferably CO
- Knowledge or education of integrative, personalized, or functional medicine a plus. Additional Benefits -
