Job Description

Caring Hospice is looking for a Nurse Practitioner for evaluating patients' initial and continued eligibility for hospice services via a Face to Face visits
Requirements :
Must be a graduate of a school accredited by the American Nurses Association. Licensed as Advance Nurse Practitioner in CT Knowledge of Medicare and Medicaid requirements outlined in the LCD guidelines for hospice eligibility. Ability to relate, assess, and treat terminally patient/families and their particular problems Good communication skills and genuine concern for patients and their families. Must have access to reliable transportation, proof of liability insurance and a U.S. driver's license. 04102026193
