Nurse Practitioner

Base line salary is supplemented by bonus structure based on productivity to achieve possible 160K plus Patient Choice Medical Care is dedicated to delivering high-quality, patient-centered healthcare services. We are looking for a full time APRN or PA to see patients primarily in the office in Hamden focusing on providing primary care services. Patient choice medical care is privately owned. Providers are well suppoted with great admnistrative staff. Responsibilities Conduct thorough patient assessments and develop individualized care plans. Perform diagnostic tests and interpret results to guide treatment decisions. Educate patients and families on health management and preventive care strategies. Collaborate with interdisciplinary teams to optimize patient care Provide support for home care services as needed, ensuring continuity of care. Maintain accurate patient records and documentation in compliance with healthcare regulations. Stay updated on best practices in nursing and healthcare trends. Qualifications Valid Nurse Practitioner license Proficiency in basic life support techniques and emergency response protocols.
