Essential Function Provide medical care to a complex patient population in the short term rehab and long term care setting Analyze patient's medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditions Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions Record in medical record the physical findings, formulated plans, and prognosis based on patient's condition within appropriate time frame Coordinate with the nursing facility care planning team in the development of comprehensive care plan Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as needed Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards Education and Experience APRN The Advanced Practice Registered Nurse is masters or doctorate prepared Certified Nationally as a Nurse Practitioner through AANP or ANCC . The Nurse Practitioner must have possession of a valid ( Connecticut ) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y ) Nurse Practitioner by an approved certifying body.
