2996662080-2 Apply for this job now Job Description Hartford Healthcare seeks a Primary Care Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services team. In this outpatient role, you will provide comprehensive primary and preventive care, manage chronic conditions, and coordinate services within our integrated network. You'll use cutting-edge technology and EMR systems, collaborate closely with physicians and an interdisciplinary team, and contribute to quality and population health initiatives. Responsibilities Provide comprehensive primary care to adult patients, including health assessments, diagnosis, and treatment Manage chronic conditions such as diabetes, hypertension, and heart disease per evidence-based guidelines Order, interpret, and act on diagnostic tests, labs, and imaging Prescribe medications and develop individualized care plans in collaboration with supervising physicians Deliver preventive care, including immunizations, screenings, and wellness counseling Educate patients and families on disease management, lifestyle changes, and treatment options Document all patient encounters accurately in the electronic medical record (EMR) Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care Participate in quality improvement, population health, and care management initiatives Precept or mentor junior providers or students as appropriate Required Skills Primary care clinical assessment and diagnosis Chronic disease management (diabetes, hypertension, COPD, CHF) Electronic Medical Record (EMR) documentation Ordering and interpreting labs and imaging Evidence-based prescribing and pharmacology Preventive care and immunization protocols Patient education and counseling Care coordination and interdisciplinary collaboration Minor office procedures (e.g., suturing, joint injections, wound care) Regulatory compliance and quality metrics (e.g., HEDIS, Medicare/Medicaid requirements)
