2996669265-2 Apply for this job now Job Description Hartford Healthcare seeks a Psychiatric Nurse Practitioner for our outpatient psychiatry team in Hartford, CT. In this role, you will provide comprehensive psychiatric evaluations, medication management, and evidence-based treatment for diverse adult and/or adolescent populations. You'll collaborate closely with psychiatrists, therapists, and primary care providers, using state-of-the-art systems and resources. Responsibilities Provide psychiatric evaluations and ongoing medication management for outpatient patients across the lifespan Develop, implement, and adjust individualized treatment plans in collaboration with psychiatrists and interdisciplinary team members Monitor treatment response, side effects, and safety, including suicide and risk assessments Educate patients and families on diagnoses, medications, and therapeutic options to support adherence and recovery Document all patient encounters, treatment plans, and clinical decisions in the electronic health record accurately and timely Coordinate care with primary care providers, therapists, social workers, and community resources Participate in case conferences, quality improvement initiatives, and clinical team meetings Maintain compliance with all regulatory, legal, and organizational policies and standards of care Required Skills Psychiatric assessment Medication management Psychopharmacology Suicide and risk assessment Diagnostic evaluation (DSM-5) Electronic Health Records (EHR) documentation Care coordination Outpatient mental health treatment Evidence-based psychiatric interventions Patient and family education
