Watertown, CT Job Details Part-time | Full-time | Per diem Up to $245,000 a year 1 day ago Benefits Flexible schedule Qualifications Collaborate with healthcare professionals Patient assessment APRN Decision making DEA License Prescribing medications Healthcare diagnosis Full Job Description Job Summary We are seeking experienced and compassionate APRNs to provide high-quality primary care services for geriatric patients in SNF's throughout CT. The ideal candidate is clinically confident, patient-centered, and experienced in managing complex chronic conditions in older adults. This provider will work collaboratively with an interdisciplinary team to deliver comprehensive, preventative, and ongoing care to our patient population. Responsibilities Provide comprehensive primary care services to geriatric patients Perform physical assessments, diagnoses, treatment planning, and follow-up care Manage chronic conditions including hypertension, diabetes, COPD, dementia, heart disease, and other age-related illnesses Conduct medication management and reconciliation Provide proactive and responsive care management with the goal of reducing hospitalizations . Order, interpret, and review diagnostic tests and lab results Coordinate care with specialists, nursing staff, families, and caregivers Educate patients and families on treatment plans, disease prevention, and wellness strategies Maintain accurate and timely documentation in the EMR system (ethizo). Participate in quality improvement and patient care initiativesQualifications Active APRN license in Connecticut Board Certification as Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or related specialty DEA license and prescriptive authority Minimum of 1-2 years of clinical experience in primary care and/or geriatrics preferred Strong assessment, diagnostic, and clinical decision-making skills Experience managing medically complex geriatric patients Excellent communication and interpersonal skills Ability to work independently while collaborating with a multidisciplinary teamPreferred Skills Experience in long-term care, skilled nursing, or geriatric practice settings Knowledge of Medicare/Medicaid guidelines Familiarity with EMR systems Strong organizational and time-management abilities Compassionate bedside manner and commitment to patient-centered care Benefits Competitive compensation Flexible scheduling Supportive clinical team environment No on call responsibilities How to Apply Please submit your resume through Indeed.
