In this role, you will provide comprehensive primary care, including health assessments, diagnosis, treatment, and patient education across a diverse patient population. You will collaborate closely with physicians and an interdisciplinary team to deliver high-quality, evidence-based care. Responsibilities Perform comprehensive patient assessments and histories Diagnose and manage acute and chronic medical conditions Order, interpret, and follow up on diagnostic tests Prescribe medications and develop evidence-based treatment plans Provide preventive care, health education, and wellness counseling Coordinate care with physicians, specialists, and ancillary staff Document care accurately in the electronic health record (EHR) Participate in quality improvement and population health initiatives Collaborate within an interdisciplinary team to optimize outcomes Maintain compliance with regulatory, safety, and infection-control standards Required Skills Primary care assessment and diagnosis Chronic disease management (e.g., diabetes, hypertension, COPD) Evidence-based treatment planning Electronic Health Record (EHR) documentation Ordering and interpreting labs and imaging Medication management and prescribing Preventive care and patient education Care coordination and referral management Clinical decision-making and triage Basic procedures (e.g., injections, simple wound care)
