Advanced Practice Clinician (NP/PA) -
ISNP Program Schedule:
Full‑time, 40 hours/week
Shift:
Monday-Friday, 8 AM-5
PM Location:
Hartford County, CT (travel between 3 facilities) Role Overview The Senior Community Care (SCC) program provides integrated, longitudinal care to high‑risk members residing in partnered facilities. The APC conducts assessments, manages chronic and acute conditions, and leads advanced care planning as part of an interdisciplinary team. This role offers work‑life balance with no on‑call, weekends, or holidays. Primary Responsibilities Conduct comprehensive assessments Manage acute and chronic medical/behavioral conditions Document accurately, including ICD‑10 Collaborate with interdisciplinary teams Lead advanced illness and care‑planning discussions Educate patients and caregivers Required Qualifications Active, unrestricted state license
NP:
Accredited MSN or DNP PA:
Accredited PA program National certification (AANP/ANCC or NCCPA) Active DEA Ability to lift 30‑lb bag, navigate stairs, and perform physical assessments Reliable transportation Collaborative practice agreement (if required) COVID‑19 vaccination Must pass pre‑employment drug screening Preferred Qualifications 1+ year clinical experience (long‑term care preferred) Experience with complex medical, behavioral, or social needs Knowledge of geriatrics, chronic illness, advanced illness, and end‑of‑life care Proficient EMR documentation
