2996666346-2 Apply for this job now Job Description Join Hartford Healthcare's Primary Care Behavioral Health team to provide integrated mental health services within a primary care setting. In this role, you'll conduct assessments, brief interventions, and care planning for diverse adult and pediatric populations. Collaborating closely with physicians, nurses, and care coordinators, you'll support evidence-based treatment, crisis intervention, and referrals to specialty care. Responsibilities Provide comprehensive behavioral health assessments within primary care clinics. Deliver brief, evidence-based interventions for common mental health and behavioral conditions. Collaborate with primary care providers and care teams to develop integrated treatment plans. Conduct risk and crisis assessments and coordinate appropriate safety or higher-level care. Educate patients and families on behavioral health conditions and self-management strategies. Document all encounters in the electronic health record in a timely, compliant manner. Coordinate referrals to specialty behavioral health or community resources as needed. Monitor treatment outcomes and participate in quality improvement initiatives.
