Monday - Friday, 8:30am-5pm. Flexible Schedule and NO on-call shifts.Travel:
The NP/PA will treat and manage Nephrology patients in the local dialysis units, office, and hospitals and requires the ability to travel throughout New Castle County, DE to these locations on a regular basis. Mileage is paid. About Nephrology Associates, P.A. Established in 1974, Nephrology Associates, P.A. is a single-specialty practice focusing on the care of patients with kidney disease and hypertension. Our physicians are on-staff at all major hospitals in Delaware, as well as Union Hospital in Cecil County, Maryland. Key Responsibilities To perform in this position successfully, an individual must be able to perform each key responsibility satisfactorily. Manage patients with chronic kidney disease, focusing on care that promotes health and slows further decline in renal function. Develop or evaluate educational materials that will allow the patient to become an active participant in his/her care and foster decision making abilities regarding treatments. Diagnose and treat dialysis related conditions and other medical conditions as appropriate. Order and interpret renal focused lab tests and diagnostic studies as indicated. Administer patient dialysis rounds in a timely manner. Support and monitor patient health maintenance activities such as health screening tests and immunizations (PNA, Flu, hepatitis). Assist in the Coordination of Care with the patient's family, dialysis facility, primary care, specialists, hospital and other health care settings as necessary Provide leadership to the clinical staff in the dialysis unit by serving as a role model and clinical resource. Access hospital records when necessary, DHIN, NAPA EMR Participate in NAPA provided educational opportunities as able, in addition to fellow lectures, journal club articles, library. Collaborate and communicate with the nephrologist to provide care and updates on patient's condition. Communicate with the patient's PCP as needed.Education and Experience Requirements Minimum Experience:
3 years of experience in a physician's office or health care setting; experience with nephrology and critical care strongly preferred.Required Licenses:
Delaware Advanced Registered Nurse Practitioner; RN License; Physician Assistant License Knowledge, Skills and Abilities Advanced knowledge of nursing and health care terminology. Work consistent with HIPPA and OSHA regulation. Experience with MS Office Suite and Electronic medical records (EMR) Excellent patient care skills. Ability to work independently but also collaboratively in a team environment. Excellent organizational and time management skills. Effectively communicate with patients, their families, and other healthcare professionals. Ability to cope with stressful situations, providing compassionate care to patients, their families, and colleagues. Proper use of personal protective equipment (PPE) and hygiene practices are required. Must be able to perform CPR. Must have reliable transportation to travel to other dialysis units, offices, and hospitals in New Castle County. Criminal background investigation and drug testing required. Physical Requirements Prolonged periods of sitting at a desk and working on a computer. May require extended periods of walking and standing. Able to lift, push, pull and carry up to 25lbs frequently, and 50lbs occasionally. Able to bend, stoop, and reach frequently. High level of manual dexterity and visual acuity is required. Adequate hearing is required for communication, cues indicating patient status, and medical equipment alarms.
