Job Description

CenterWell Home Health, part of Humana Inc., seeks a Nurse Practitioner or Physician Assistant to deliver high-quality, in-home care to seniors and adults with chronic conditions. In this role, you'll conduct comprehensive assessments, develop and manage individualized care plans, prescribe and adjust medications, and coordinate with an interdisciplinary team to support recovery and independence at home. You'll educate patients and caregivers, use electronic health records for documentation, and ensure compliance with safety and regulatory standards. Responsibilities Provide in-home primary and episodic care to adult and senior patients Conduct comprehensive assessments, develop and adjust care plans Collaborate with physicians, therapists, and interdisciplinary team members Prescribe and manage medications per state scope of practice Order and interpret diagnostic tests and lab results Educate patients and caregivers on disease management and prevention Document visits and care plans in the electronic health record Monitor patient progress, respond to changes in condition, and coordinate follow-up care Ensure compliance with clinical protocols, safety, and regulatory standards Participate in quality improvement and case conferences Required Skills Advanced patient assessment and diagnostic skills Care planning and chronic disease management Medication management and prescribing Wound care and post-acute care management Use of electronic health records (EHR) Ordering and interpreting lab and diagnostic tests Patient and caregiver education Interdisciplinary care coordination Knowledge of home health regulations and documentation standards Time management and autonomous practice in the field
