Our team provides medical care to patients in skilled nursing facilities, post-acute rehabilitation settings, and assisted living communities , with a strong emphasis on continuity, communication, timely follow-up, and evidence-based medicine. We care for a medically diverse population, including older adults with chronic medical conditions, patients recovering after hospitalization, and residents requiring ongoing management in senior-living communities. We use a team-based, technology-forward approach to help providers work efficiently while maintaining high-quality patient care. Our physicians, advanced practice providers, clinical staff, and facility teams communicate closely to coordinate care and address changes in patient condition promptly. Our practice also routinely trains medical and physician assistant students. Teaching, continuing education, and process improvement are important parts of our culture. Position Overview We are seeking a conscientious, dependable, and motivated Nurse Practitioner or Physician Assistant to join our team caring for patients in skilled nursing, rehabilitation, and assisted living facilities. This position is ideal for a provider who enjoys internal medicine, geriatrics, post-acute care, and longitudinal patient relationships . The provider will evaluate and manage patients independently within their scope of practice while working closely with the supervising/collaborating physician and interdisciplinary facility teams. While previous SNF, ALF, geriatric, or internal medicine experience is valued, we are also willing to train the right new graduate who demonstrates strong clinical judgment, reliability, communication skills, and a genuine willingness to learn. Comprehensive on-the-job training and physician support are provided. Responsibilities Round on patients in skilled nursing facilities, rehabilitation facilities, and assisted living communities . Perform comprehensive and focused medical evaluations. Manage acute and chronic medical conditions. Evaluate changes in condition and determine appropriate diagnostic and treatment plans. Follow patients after hospitalization and during post-acute rehabilitation. Review laboratory results, imaging, medications, nursing concerns, and consultant recommendations. Perform medication reconciliation and help reduce unnecessary or potentially harmful medications when appropriate. Manage common geriatric conditions including hypertension, diabetes, CHF, COPD, infections, falls, cognitive impairment, and functional decline. Coordinate care with physicians, nursing staff, therapists, specialists, families, pharmacies, and other members of the interdisciplinary team. Communicate clearly and compassionately with patients and family members. Provide patient and family education regarding diagnoses, treatment plans, prognosis, and goals of care. Complete timely and accurate documentation in the electronic medical record. Respond appropriately to facility messages and patient-care concerns during assigned working hours. Participate in equitable after-hours phone/secure-message call coverage as scheduled. Participate in quality improvement, clinical education, and development of efficient care processes. Graduate of an accredited Nurse Practitioner or Physician Assistant program. Current national certification appropriate to profession. DEA registration or eligibility to obtain one. Strong clinical assessment and patient-management skills. Excellent written and verbal communication skills. Comfortable using electronic medical records and secure digital communication systems. Ability to travel locally between assigned facilities. Reliable transportation required.
