Boca, Clewiston and West Palm About the Role PHA is seeking a dedicated PMHNP to join our growing team. You will provide psychiatric care across multiple skilled nursing facilities, collaborating with staff and ensuring the highest quality of patient-centered care. This position offers autonomy, professional growth, and the support of an experienced physician-led group. Primary Responsibilities Conduct psychiatric evaluations, record patient histories, and maintain accurate documentation. Develop and implement individualized treatment plans in collaboration with supervising physicians and facility staff. Order and interpret diagnostic tests as needed. Prescribe and manage psychiatric medications. Educate patients on health promotion, disease prevention, and psychiatric conditions. Refer patients to specialists when appropriate. Participate in case reviews, consultations, and ongoing care coordination. Adhere to regulatory requirements and reporting standards. Additional duties as assigned. Qualifications Active State of Florida APRN license in good standing. National Board Certification as
PMHNP. DEA
license required. Post-acute care experience required . 1 year of psychiatric experience preferred (training available for the right candidate). Proficiency with electronic medical records (EMR). Excellent communication, problem-solving, and teamwork skills. Bilingual (English/Spanish) a plus but not required.
