Job Summary - Horizon Medical Clinic We are seeking a highly motivated and compassionate Nurse Practitioner - Advanced Practice Registered Nurse (APRN) to join our dynamic healthcare team. In this vital role, you will deliver comprehensive patient care across diverse settings, utilizing your expertise to assess, diagnose, and treat a wide range of medical conditions. Your proactive approach and clinical knowledge will contribute to improving patient outcomes, promoting health education, and supporting collaborative healthcare delivery. Duties Conduct thorough physical examinations and patient assessments, including vital signs and medical history documentation using Electronic Health Record (EHR) . Prescribe medications, administer injections, and perform procedures like suturing, venipuncture, catheterization. Manage acute and chronic conditions across various specialties including gastroenterology, geriatrics, behavioral health, infectious disease care, . Collaborate with healthcare professionals to develop treatment plans, coordinate care for patients in settings such as nursing homes, assisted living facilities, hospitals . Utilize telehealth platforms to provide remote consultations and follow-up care while maintaining compliance with all healthcare laws and standards. Participate in clinical research initiatives and utilize diagnostic tools like sonography and laboratory testing to support accurate diagnosis. Engage in patient education on health coaching topics such as nutrition, medication management, wound care (including sterile processing), and disease prevention strategies. Skills Extensive experience working with Electronic Health Records (EHR) systems such ECW. Proficiency in performing advanced procedures including botulinum toxin treatment, dermal fillers, spinal taps, tube feeding management, and esthetic laser treatments. Strong background in critical care environments including ICU, emergency room (ER), trauma centers (Level I & II), PICU experience, and hospital medicine. Knowledge of diverse medical disciplines such as internal medicine. Skilled in performing diagnostic evaluations using CPT coding for procedures like cardiac catheterization or catheterization procedures; ICD coding for diagnoses;
ICD-9 & ICD-10
familiarity for documentation accuracy. Ability to perform physical examinations including airway management, venipuncture, IV insertion/infusion therapy, specimen collection/processing, phlebotomy, sterile technique & aseptic procedures. Experience working with complex patient populations including individuals with developmental disabilities or those requiring post-acute care or hospice services. Excellent client communication skills with the ability to educate patients on treatment plans while ensuring compliance with HIPAA standards.
