In this vital role, you will provide comprehensive primary care services, perform advanced clinical assessments, and develop personalized treatment plans. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform diagnosis and treatment strategies. Perform diagnostic evaluations utilizing EMR system eMedicalPractice; interpret laboratory results, imaging studies, and diagnostic tests including sonography and gastroscopy. Administer medications and injections responsibly while educating patients on medication management, pain control options—including acute pain management—and health coaching. Collaborate with multidisciplinary teams on discharge planning, case management, telehealth consultations, and utilization review to optimize patient outcomes across inpatient and outpatient settings. Provide specialized care in areas such as geriatrics, hospice & palliative medicine, behavioral health, infectious disease care, occupational health, and family planning. Engage in clinical research initiatives or quality improvement projects to advance evidence-based practices within the organization. Qualifications Current certification as an Advanced Practice Registered Nurse (APRN) with family medicine specialization; valid state licensure required. Demonstrated experience with EMR/EHR systems such as Epic, Cerner or Athenahealth; proficiency in CPT coding and ICD coding (ICD-9/10). Excellent communication skills for triage calls, patient education on topics like nutrition or dermal fillers, and collaborating effectively within multidisciplinary teams.
