Practice Hours:
Monday-Friday, 8:30 a.m.-5:30 p.m Position Summary The Nurse Practitioner will assess, evaluate, diagnose, treat, and manage adult and senior patients. The provider will deliver a full spectrum of primary and preventive healthcare services, including acute care, chronic disease management, annual wellness visits, preventive examinations, same-day visits, and follow-up care. The provider will develop comprehensive treatment plans that may include pharmacological and non-pharmacological interventions, patient and family education, lifestyle recommendations, preventive care, and appropriate referrals to physicians, specialists, and other members of the healthcare team. The selected provider must be able to safely and efficiently manage an average of 15-20 patients per day, based on patient complexity and scheduled hours, without compromising patient safety, quality of care, documentation accuracy, or the patient experience. Essential Responsibilities Perform comprehensive and focused histories and physical examinations for adult and senior patients. Evaluate, diagnose, and treat acute illnesses, injuries, and chronic medical conditions within the provider's education, training, certification, and scope of practice. Manage new-patient visits, same-day appointments, annual physicals, Medicare Annual Wellness Visits, hospital or emergency-room follow-ups, and chronic care visits. Develop individualized treatment plans using current evidence-based clinical guidelines and nationally recognized standards of care. Manage common chronic conditions, including diabetes, hypertension, hyperlipidemia, obesity, asthma, COPD, and other conditions commonly treated in primary care. Order, review, and accurately interpret laboratory tests, diagnostic imaging, screenings, and other diagnostic studies. Communicate abnormal or time-sensitive results promptly and document all follow-up actions. Recognize changes in a patient's condition and make timely referrals to a physician, specialist, emergency department, or other appropriate level of care. Perform medication reconciliation and monitor medication effectiveness, adherence, contraindications, interactions, and potential adverse effects. Identify and address applicable HEDIS measures, preventive screenings, diagnosis gaps, and care opportunities during each patient encounter. Accurately capture relevant diagnoses, disease severity, and HCC risk-adjustment information based on conditions evaluated and supported by the medical record. Follow all coding, billing, and documentation requirements. Complete visit documentation, orders, coding, and billing entry accurately and within the organization's required timeframe, preferably on the date of service. Review and respond promptly to laboratory results, refill requests, patient messages, clinical tasks, and assigned electronic health record inbox items. Educate patients and families regarding diagnoses, medications, treatment plans, lifestyle modifications, preventive care, and follow-up recommendations. Promote continuity of care by ensuring that referrals, diagnostic testing, and follow-up services are properly documented and addressed. Collaborate respectfully with physicians, clinical leadership, medical assistants, front-office staff, and other members of the healthcare team. Participate in quality-improvement initiatives, clinical training, staff meetings, and performance-monitoring activities when required. Follow organizational policies, infection-control standards, HIPAA requirements, and all applicable federal and state regulations. Maintain a professional, compassionate, culturally responsive, and nonjudgmental approach with every patient. Remain flexible and willing to assist the team when operational or patient-care needs arise. Minimum Requirements Current and unrestricted Florida Advanced Practice Registered Nurse license. Current national board certification as one of the following:
Family Nurse Practitioner:
FNP-C or FNP-BC; or
Adult-Gerontology Primary Care Nurse Practitioner:
AGPCNP-C or
AGPCNP-BC.
Current BLS certification. Ability to provide comprehensive primary care to adult and senior patients. Minimum of two years of direct primary care experience preferred. Professional proficiency in both English and Spanish is required. Working knowledge of HEDIS measures, preventive care requirements, HCC coding, risk adjustment, and accurate diagnosis documentation. Strong knowledge of current evidence-based adult and geriatric primary care guidelines. Experience using electronic health record systems; athenaOne/athenahealth experience is strongly preferred. Excellent clinical documentation, written communication, and interpersonal skills. Strong organizational skills and exceptional attention to detail. Ability to work independently while recognizing when physician consultation, collaboration, or escalation is necessary. Ability to delegate appropriately and make sound clinical judgments with patient safety as the highest priority. Ability to work effectively in a dynamic and fast-paced clinical environment. Current DEA registration and controlled-substance prescribing credentials, if required for assigned responsibilities. Ability to meet all credentialing, background-screening, and professional liability requirements.
