Job Description

Conviva Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive primary care to adults 65+. In this role, you'll provide assessments, chronic disease management, preventive care, and same-day visits within a coordinated, value-based model. You'll collaborate with physicians, nurses, and care teams to develop personalized care plans, monitor outcomes, and support wellness programs that promote independence and quality of life. Responsibilities Provide comprehensive primary care to adults 65+, including assessments, diagnosis, and treatment. Manage chronic conditions through evidence-based care plans and regular follow-up. Deliver preventive services, wellness visits, and health screenings. Prescribe and manage medications in accordance with clinical guidelines and regulations. Review and interpret labs, imaging, and diagnostic results to guide care decisions. Document care accurately in the Electronic Medical Record (EMR) and track quality metrics. Collaborate with physicians, nurses, and care team members to coordinate value-based care. Educate patients and families on conditions, medications, and lifestyle modifications. Participate in quality improvement initiatives and population health efforts. Engage in ongoing professional development and contribute to a patient-first, team-based culture.
