Job Description

Job Summary Serene Resolutions is hiring a Psychiatric Mental Health Nurse Practitioner to join our growing mobile behavioral-health team. This role supports adult and geriatric patients in assisted living, skilled nursing, and other facility-based settings by bringing psychiatric care directly to patients where they live. This is a strong fit for a psychiatric provider who enjoys practical, relationship-based clinical work with older adults, patients with cognitive decline, families, and facility staff. Our psychiatric team works closely with therapists, care coordinators, facility teams, and support staff to improve continuity of care for patients in real-world facility settings.
Compensation:
$145,000-$170,000 per year, depending on experience, credentialing status, schedule, facility coverage, and visit volume. Contractor or per-visit arrangements may also be considered based on an agreed fee schedule. Responsibilities Provide psychiatric evaluations and medication-management follow-up visits for adult and geriatric patients in facility-based settings. Support patients dealing with depression, anxiety, mood disorders, behavioral symptoms, cognitive decline, grief, adjustment issues, and chronic medical challenges. Assess psychiatric symptoms, medication response, side effects, safety concerns, and changes in functioning. Adapt care for patients with dementia, memory loss, limited insight, medical complexity, or difficulty participating in traditional outpatient care. Coordinate with therapists, facility staff, families, primary care providers, and internal Serene team members when clinically appropriate. Communicate concerns clearly and professionally to the appropriate care team members. Complete timely, accurate clinical documentation in the company's EHR. Maintain HIPAA-compliant communication and documentation practices. Participate in onboarding, workflow training, and ongoing development as Serene expands its behavioral-health program. Help build a practical, patient-centered psychiatric care model that works in real facility environments. Qualifications Active Florida APRN license. Active board certification as a Psychiatric Mental Health Nurse Practitioner. Experience providing psychiatric care to adults, older adults, or medically complex patients preferred. Comfort working with geriatric patients, dementia-related behaviors, family concerns, facility-based care environments, and medication-management needs. Strong clinical judgment and the ability to adapt care to each patient's cognitive, emotional, and medical presentation. Clear, professional communication style with patients, families, facility staff, and internal team members. Ability to work independently while staying coordinated with the broader care team. Strong documentation habits and willingness to complete notes promptly after visits. Reliable transportation for facility-based work. Telehealth experience is helpful but not required.
