Job Description
This is not intended to be simply a supporting role to the physician. The Nurse Practitioner will have the opportunity to develop and manage their own patient panel while collaborating closely with Dr. Clark on more complex patients when appropriate. The NP will also play an important role in maintaining continuity of care and appointment availability during periods when Dr. Clark is away from the office practicing Interventional Cardiology. We are looking for someone who values thoughtful, relationship-based medicine and wants to become an important part of a growing independent practice. Responsibilities Provide comprehensive outpatient Primary Care and Internal Medicine services to adult patients Evaluate, diagnose and manage acute and chronic medical conditions Perform annual wellness, preventive and routine follow-up visits Manage chronic conditions such as hypertension, diabetes, hyperlipidemia and other common adult medical conditions Perform medication management and reconciliation Provide post-hospital and transitional-care follow-up Order and interpret appropriate laboratory and diagnostic testing Develop individualized treatment and preventive-care plans Educate patients regarding their medical conditions, medications, prevention and lifestyle Maintain accurate and timely documentation in the electronic health record Coordinate care with specialists, hospitals and other healthcare providers Develop long-term relationships with patients and their families Collaborate with Dr. Clark and the clinical team regarding complex patients when appropriate Help maintain continuity and appointment availability when Dr. Clark is away from the office
