In this dynamic role, you will deliver comprehensive healthcare services directly to patients in their homes. Your expertise will help bridge gaps in care by providing accessible, high-quality medical attention where it's needed most. This position offers an exciting opportunity to work autonomously while making a meaningful difference in diverse patient populations across multiple locations. If you thrive on mobility, patient engagement, and clinical excellence, this role is perfect for you! Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews across different community settings. Conduct Anual Wellness Visits. Manage acute and chronic conditions with a focus on geriatrics, behavioral health, pain management, and infectious disease care. Refer out as needed. Collaborate with multidisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure seamless continuity of care. Utilize electronic health record (EHR) system Athenahealth and medical apps for accurate documentation and medical records management. Educate patients and their families on health coaching, medication administration, nutrition, and disease prevention strategies. Skills Proven experience working with diverse populations including seniors, and those requiring hospice or palliative care. Strong proficiency in EMR/EHR systems like Athenahealth; familiarity with CPT coding, ICD-9/10 coding is highly desirable. Skilled in performing procedures such as phlebotomy, catheterization, sterile processing techniques, and aseptic techniques. Excellent communication skills for patient education and interdisciplinary collaboration; understanding of HIPAA regulations is essential. Certifications in Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), and other relevant credentials required.
