In this role, you will provide high-quality, patient-centered care to individuals with acute and chronic health conditions, including those with developmental disabilities. You will work collaboratively within a multidisciplinary team to support patients in achieving optimal health outcomes and an improved quality of life. This position also offers opportunities for leadership growth and advancement within the organization for motivated candidates. Key Responsibilities Perform comprehensive health assessments to identify the medical needs, goals, and challenges of individuals with developmental disabilities Develop, implement, and evaluate individualized, evidence-based treatment plans Provide direct clinical care, including support with activities of daily living as appropriate Administer medications and treatments in accordance with established protocols and provider orders Collect, process, and document specimens for laboratory testing Collaborate closely with physicians, nurses, therapists, and other healthcare professionals to coordinate care and monitor patient progress Participate in discharge planning and facilitate smooth transitions to community-based resources and services Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory and organizational standards What We Offer Competitive compensation with a salary range of $100,000 - $150,000 Clear pathways for leadership, management, and clinical advancement Supportive, team-oriented work environment Opportunity to make a meaningful impact in the lives of individuals and families Ongoing professional development and mentorship opportunities
