    General Description:
    The APRN is responsible for promoting and demonstrating Parrish Healthcare's Culture of Choice®; ensuring clinical quality as well as strategic goals and objectives are met or exceeded according to State of Florida regulatory guidelines for nurse practitioners. The APRN will effectively fulfill our mission to provide Healing Experiences for Everyone all the Time® and achieve organizational strategic (Game Plan) goals. This position reports to the AVP, Parrish Medical Group Operations and supervising provider.
    Key Responsibilities:
    PATIENT CARE
    1. Functions independently by implementing appropriate evidence-based standardized procedures/protocols and by effectively manages a panel of patients; Conducts comprehensive assessments of patients; gathers health history, performs physical examinations, appropriately prescribes medications, effectively diagnoses and treats illnesses/injuries, ensures annual wellness milestones are met (e.g. screenings and vaccinations); and ensures compliance with CMS quality metrics related to MIPS/HEIDS, etc. 2. Notifies managing physician about unusual or unexpected events or problems; makes appropriate referrals to other healthcare professionals and support services 3. Dictate and document all patient interactions and services rendered per medical record completion policy 4. Provides education and support to caregivers on dementia, its progression, and coping strategies to include healthy lifestyle behaviors, disease prevention and overall health promotion 5. Support at-home rounding support for home bound patients, nursing or rehab facilities, to provide initial or follow up diagnoses, to include provider referral management for monitoring disease progression and or management.

    Provides initial and ongoing patient assessments, including patient's physical, psychological, and environmental factors. Uses assessment information from the patient assessment to develop a recommended plan of care to be reviewed and approved by the responsible physician. This is done in accordance with the State of Florida Nursing Act for Nurse Practitioners. 9. Prescribes medication, examine patients, diagnose illnesses and provide treatment; Order, interpret, and evaluate diagnostic tests; Gather health histories, perform physical examinations and diagnostic procedures 10. Develops and implements treatment plans; Dictate and document all patient interactions; Notify managing physician about unusual or unexpected events or problems. 11. Functions independently by implementing appropriate standardized procedures. 12. Using standardized procedures, changing treatment regimen after observing signs and symptoms of illness, reactions to treatment, general behavior and physical condition and determines if they are abnormal findings. 13. Provides and coordinates the care of patients, to include providing patient/family education promoting safe continuity of care. 14. Participates in interdisciplinary and clinical rounds, provides education and support, as well as making appropriate referrals and documents services rendered per medical record completion policy. 15. Effectively communicates with diverse populations, including physicians, employees, patients, and families. 16. Ensures the adherence to systems, processes, policies and procedures that result in a continuous improvement environment within which assigned patients always receive national best/leading-practice standards for quality and excellence as measured by HIPAA, CMS, Joint Commission, and other regulatory agencies pertinent to areas of responsibility; holding self and others accountable for actions and outcomes. 17. Knows fire, disaster and safety procedures and regulations as pertains to the work area.

Performs other duties as assigned.
    Requirements:
    Formal Education:
    Masters in Nursing required, Doctorate of Nursing preferred. Nurse Practitioner (ARNP) program required.
    Work Experience:
    Two or more years of clinical experience as a nursing practitioner at a physician practice or in a hospital with a related specialty is strongly preferred.
    Required Licenses, Certifications, Registrations:
    Florida Registered Advanced Nurse Practitioner required. National Provider Identifier (NPI) and Drug Enforcement Administration (DEA) number required. BLS and ACLS through The American Heart Association required.
