Job Description

Position Overview:
We are seeking a full-time Family Nurse Practitioner to serve as a float provider across multiple clinic location. This role includes a structured weekly schedule with designated days at specific offices, along with occasional coverage at additional locations as needed.
Working Hours:
Monday through Friday, 8:30 AM - 5:00 PM No weekends or evening shifts required.
Job Responsibilities:
Provide comprehensive primary care services across the lifespan, including health assessments, diagnosis, and management of acute and chronic conditions. Float between assigned clinic locations on a set weekly schedule, with occasional adjustments to support coverage needs. Order, interpret, and follow up on diagnostic tests; prescribe medications and treatments in accordance with scope of practice. Develop and implement evidence-based treatment and preventive care plans, including immunizations, screenings, and patient education. Collaborate with physicians and interdisciplinary team members to ensure high-quality, patient-centered care across all sites. Document patient encounters accurately in the electronic health record while maintaining compliance with regulatory and organizational standards. Promote health literacy and support patients and families in managing their health and wellness.
