Job Description

Job Overview We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this role, you will deliver comprehensive primary and specialized care to diverse patient populations, including pediatrics, geriatrics, and individuals with complex medical needs. Your expertise will support patient health through thorough assessments, diagnosis, treatment planning, and ongoing management. All roles are compensated and require a dedicated professional committed to providing exceptional patient care. Responsibilities Conduct detailed physical examinations and health assessments across various age groups, including pediatric and geriatric patients. Prescribe medications in accordance with state regulations and collaborative practice agreements, ensuring compliance with DEA regulations. Manage acute and chronic conditions such as infectious diseases, pain management issues, gastrointestinal concerns, and behavioral health needs. Perform diagnostic evaluations utilizing EMR/EHR systems like Elation Health to document patient encounters accurately. Collaborate closely with physicians, specialists, nurses, and other healthcare professionals to develop comprehensive care plans. Provide patient education on disease prevention, medication administration, nutrition, and lifestyle modifications to promote optimal health outcomes. Experience Proven experience working in diverse healthcare settings such as hospitals, outpatient clinics, urgent care centers, or nursing homes. Demonstrated proficiency with EMR/EHR systems for medical documentation and patient management. Strong background in managing chronic care management Experience working with children (pediatrics), seniors (geriatrics), or individuals with disabilities including developmental disabilities or dementia enhances your candidacy. Knowledge of CPT coding, ICD-9/10 coding systems for billing and documentation purposes is essential. Pay:
$90,000.00 - $125,000.00 per year
