Position Highlights 5 days per week Weekdays only No nights No weekends On-site clinical hours approximately 9:00 AM-4:00 PM Flexibility to complete documentation from home Potential annual compensation of $110,000-$160,000 , based on productivity Paid Time Off (PTO) Paid sick time $1,000 CME reimbursement Family-friendly work environment Physician support available Opportunity for a high degree of clinical independence What You'll Do As the Advanced Practice Provider, you will serve as the consistent, visible clinical presence in the facility and provide direct medical care to a short-stay, post-acute rehabilitation population and Long term care facilities. Our patients may include individuals recovering from: Orthopedic procedures and injuries Cardiac conditions Pulmonary illness Stroke Neurologic conditions Other complex medical conditions requiring post-acute rehabilitation Key Responsibilities Provide comprehensive assessment, diagnosis, treatment, and follow-up for post-acute patients. Round proactively on patients and respond to clinical needs in a timely manner. Meet new admissions, establish expectations for the patient's stay, and communicate with patients and families. Manage acute and chronic medical conditions in the facility whenever it is safe and clinically appropriate to do so. Help reduce avoidable emergency department transfers and hospitalizations through proactive management. Collaborate with facility leadership, nursing, therapy teams, physicians, specialists, and families. Develop and update individualized care plans. Monitor patient progress throughout the rehabilitation stay. Communicate effectively with patients, families, nursing staff, and interdisciplinary team members. Maintain accurate and timely clinical documentation in the EHR. Provide patient-centered care while maintaining appropriate clinical and regulatory standards.

Compensation Potential annual compensation:

$110,000-$160,000 Compensation is based on the position's compensation structure and productivity . QualificationsRequired Current Nurse Practitioner or Physician Assistant license in the State of Georgia NP candidates: Board certification in Internal Medicine, Geriatrics, or Family Practice PA candidates: Current NCCPA certification Unrestricted DEA license Ability to practice independently in a facility setting with physician support Familiarity with EHR systems and clinical documentation best practices Preferred 1-2 years of NP or PA experience Experience in subacute rehabilitation, post-acute care, skilled nursing, geriatrics, or transitional care Experience working in an inpatient or high-acuity skilled nursing facility setting Strong clinical judgment and ability to manage patients with multiple medical conditions Excellent communication and collaboration skills Why Join Us?
