Job Description
Nurse Practitioner (NP) - Primary Care Horizon Med Clinic is seeking a friendly, motivated Nurse Practitioner (NP) to join our growing outpatient primary care practice. We are looking for a provider who enjoys developing long-term relationships with patients and wants to work in a supportive, patient-focused environment with a high degree of clinical autonomy. Responsibilities Provide primary care for adult patients Perform annual physicals and wellness visits Evaluate and treat acute illnesses and minor injuries Diagnose and manage common chronic conditions including hypertension, diabetes, high cholesterol, and thyroid disorders Order and interpret laboratory and diagnostic testing Prescribe medications and develop individualized treatment plans Provide preventive care and health screenings Manage follow-up visits and medication refills Refer patients to specialists when appropriate Maintain accurate and timely medical documentation Educate patients regarding medications, treatment plans, and healthy lifestyle choices Qualifications Active Georgia Nurse Practitioner license Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care NP preferred Current prescriptive authority Strong communication and patient-care skills Ability to practice independently within applicable Georgia requirements Primary care experience preferred, but qualified candidates are encouraged to apply What We Offer Competitive compensation Consistent outpatient schedule No hospital rounds Supportive, locally operated practice Clinical autonomy Opportunity to grow with the practice Friendly and professional work environment Schedule Monday-Friday No hospital rounding.
