Assisted Living, Independent Living, and Memory Care Communities Position Type:
Full-Time Position Summary Swan Primary Care is seeking a compassionate and dependable Nurse Practitioner or Physician Assistant to provide on-site primary care services to residents of assisted living, independent living, and memory care communities throughout Chicago's northwest suburbs. The provider will independently manage a panel of primarily geriatric patients while collaborating with facility nursing staff, families, specialists, pharmacies, home health agencies, and supervising physicians. The ideal candidate is comfortable working autonomously, managing medically complex patients, and traveling between senior living communities. Primary Responsibilities Conduct scheduled on-site rounds at assigned assisted living, independent living, and memory care communities. Perform comprehensive new-patient evaluations, routine follow-up visits, and annual wellness examinations. Diagnose and manage acute and chronic medical conditions, including hypertension, diabetes, heart failure, COPD, chronic kidney disease, dementia, depression, and other common geriatric conditions. Evaluate residents for changes in condition, falls, confusion, behavioral changes, infections, wounds, weight changes, functional decline, and medication-related concerns. Develop and update individualized treatment plans based on each resident's medical condition, functional status, goals of care, and living environment. Order and review laboratory testing, diagnostic imaging, medications, therapy services, and specialty referrals when clinically appropriate. Perform medication reconciliation and reduce unnecessary or potentially harmful medications when appropriate. Monitor high-risk medications and follow laboratory results to ensure safe treatment. Coordinate care following emergency department visits, hospitalizations, rehabilitation stays, and transitions back to the facility. Identify residents who may benefit from home health, physical therapy, occupational therapy, palliative care, or hospice services. Discuss goals of care, advance directives, code status, and changes in prognosis with residents and authorized family members when appropriate. Communicate significant findings and treatment recommendations promptly to facility nursing staff, patients, families, and the supervising or collaborating physician. Provide education to residents, families, and caregivers regarding diagnoses, medications, treatment plans, and disease prevention. Complete accurate and timely documentation for all patient encounters. Select appropriate diagnoses and billing levels while maintaining compliant clinical documentation. Review and respond to laboratory results, facility messages, refill requests, and other clinical communications within established timeframes. Participate in quality-improvement initiatives focused on fall prevention, medication safety, chronic-disease management, and reducing avoidable emergency department visits and hospital readmissions. Maintain professional and collaborative relationships with facility administrators, nursing teams, pharmacies, specialists, and community partners. Follow HIPAA requirements, organizational policies, facility protocols, and applicable federal and Illinois regulations. Participate in periodic provider meetings, case reviews, and clinical training. Travel reliably between assigned facilities within the northwest suburban territory. Qualifications Current Illinois Nurse Practitioner or Physician Assistant license in good standing. Current controlled-substance license and DEA registration, or eligibility to obtain them. Board certification as an NP or PA. Graduate of an accredited Nurse Practitioner or Physician Assistant program. Current BLS certification. Ability to work independently while recognizing when physician consultation or a higher level of care is necessary. Strong clinical judgment, communication, organizational, and time-management skills. Reliable transportation and willingness to travel between senior living communities. Proficiency with electronic health records and mobile documentation. Geriatric, primary care, assisted living, memory care, SNF, home-based medicine, or hospital medicine experience is preferred. New graduates with strong geriatric or primary care training may be considered.
