Job Description

Nurse Practitioner - Provider | Bradley, IL & Crandall, IL Hometown NP - Bradley, Illinois. Crandall, Texas Part-Time → Full-Time Growth Opportunity Hometown NP is seeking a motivated Nurse Practitioner to serve as a Provider for our growing Bradley, Illinois practice. This position is designed for an NP who wants to establish their own patient panel and grow with the practice. The position will begin 2 days per week and expand to 3-4 days per week as patient volume increases . Compensation W-2 position $40/hour guaranteed base +$60 per completed initial evaluation +$30 per completed follow-up Additional productivity incentive for qualifying psychotherapy services No productivity bonus for cancellations or no-shows Performance and long-term retention bonus opportunities Increased days and earning potential as the patient panel grows Responsibilities Provide outpatient psychiatric evaluations, medication management, follow-up care, treatment planning, appropriate psychotherapy, care coordination, and timely clinical documentation. The provider will also play an important role in building and retaining their patient panel, developing community relationships, and helping establish Hometown NP in the Bradley/Bourbonnais area or Crandall/Forney area. Qualifications Licensed Nurse Practitioner in Illinois or Texas
PMHNP-BC
preferred/required depending on scope Prescriptive authority as applicable Strong documentation and patient-care skills Dependable and comfortable practicing independently Interested in growing with a new practice Must live within reasonable commuting distance of
Bradley/Bourbonnais, Illinois Or Crandall/Forney Texas Schedule:

$117,095.39 - $141,018.11 per year
