Champaign, IL Job Details $90,000 - $110,000 a year 10 hours ago Qualifications Direct patient care Cultural competency Nurse Practitioner (NP) Advanced practice provider Working with individuals from diverse cultural backgrounds Patient interaction Cross-functional communication Full Job Description Nurse Practitioner Community Health Partnership of Illinois (CHP) is seeking a compassionate and dedicated Nurse Practitioner (NP) to join our healthcare team. The Nurse Practitioner will provide comprehensive, patient-centered primary care services to individuals and families while supporting CHP's mission of providing quality healthcare to the communities we serve. Responsibilities Provide comprehensive primary care services to patients across the lifespan. Conduct physical examinations, health assessments, and routine preventive care. Diagnose and manage acute and chronic health conditions. Order, perform, and interpret appropriate diagnostic and laboratory tests. Develop and implement individualized treatment and care plans. Prescribe medications and treatments within the scope of practice. Educate patients regarding diagnoses, medications, preventive care, and healthy lifestyle practices. Maintain accurate, timely, and complete documentation in the electronic health record (EHR). Collaborate with physicians, nurses, clinical staff, and other healthcare professionals to coordinate patient care. Make appropriate referrals to specialists and community resources when necessary. Follow CHP clinical policies, procedures, quality standards, and regulatory requirements. Participate in quality improvement and population health initiatives. Perform other related duties as assigned. Qualifications Master's or Doctoral degree from an accredited Nurse Practitioner program. Current and unrestricted Illinois Advanced Practice Registered Nurse (APRN) license or eligibility to obtain one. Current national Nurse Practitioner certification from a recognized certifying organization. Current Registered Nurse (RN) license in Illinois, as required. Current DEA registration or eligibility to obtain registration, if prescribing controlled substances is required for the position. Current BLS/CPR certification. Strong clinical assessment, diagnostic, and patient-care skills. Excellent communication, organizational, and interpersonal skills. Ability to work effectively with patients from diverse cultural and socioeconomic backgrounds. Experience in primary care, community health, or a Federally Qualified Health Center (FQHC) environment is preferred. Bilingual skills are a plus.
