PART-TIME MID LEVEL PROVIDER
(FAMILY NURSE PRACTITIONER or
PHYSICIAN ASSISTANT
).
Responsibilities:
Provide quality medical care to all patients with physician supervision as needed.

Perform physical examinations, record medical histories, and formulate accurate diagnosis under physician's supervision. Provide continuity of care for patients by instructing patient and family regarding medications and treatment instructions. Review laboratory, radiology, and other patient testing results.

Complete patient charts and provide adequate documentation.
Required:
MUST HAVE
Current Illinois Certificate/License MUST be able to work
INDEPENDENTLY MUST
be able to work all
SATURDAYS
from 9:00 a.m. - 3:00 p.m.
