Job Description

Provide high-quality, patient-centered care to residents in skilled nursing facilities in Cahokia and Hillsboro, IL, with additional openings in MA, MI, NH, NC, OH, and WI. Location and Schedule Full-time positions with flexible scheduling Six-hour facility shifts; rounds typically begin between 7:00 a.m. and 10:00 a.m. No on-call or overnight responsibilities Core Responsibilities Perform daily rounds in skilled nursing and post-acute care settings Manage acute and chronic conditions; prescribe medications and treatment plans Document patient encounters using a streamlined EHR with AI scribe and voice dictation Use company-provided iPad with LTE and access UpToDate® for clinical guidance Collaborate with interdisciplinary care teams to optimize resident outcomes Compensation and Benefits Competitive base pay with uncapped productivity-based earning potential Up to $50,000 in long-term retention incentives Performance bonuses and sign-on bonus (eligibility applies) Paid Time Off 401(k) with employer match Medical, dental, vision, life, and disability insurance (eligibility applies) CME allowance and licensure reimbursement Company-provided iPad with LTE and lab coat Relocation assistance available for eligible candidates Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license National certification (ANCC, AANPCB, PNCB, AACN, NCCPA, or equivalent) Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
