POSITION SUMMARY
Pilsen Medical Clinic & Associates is actively seeking Independent Contractor (APNP) Advanced Practice Nurse Practitioner to complete home visits around the Chicagoland area. We invite you to choose a career with our growing home visiting physician team! As a team we work together to provide primary care services for seniors in their homes. Our goal is to provide seniors with the highest quality medical services in the greater Chicago area, emphasizing wellness and management of chronic diseases. Each practitioner is partnered with a medical assistant to ensure exceptional, quality care for each patient. Very competitive compensation package offered. Flexible Schedule ( You can work as minimal as 4 days per month.)
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. Other duties may be assigned . Provide exemplary customer service to patients, visitors and coworkers. Provide convenient, friendly patient care to all seeking health related services. Performs medical assessment of patients Complete home visit Encounter Summary within 48 hours of appointment Assures compliance with licensing, certification and accrediting bodies. Represent company and the clinic as reasonably required at internal and external meetings or events. Maintain working equipment and supplies. Coordinate patient care with patient's associated home health agency Review patient case document in an timely manner. Full time or part time available
