New graduates welcome We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric care to geriatric patients in nursing homes and skilled nursing facilities within 5 miles of one another in Joliet, IL . This is a great opportunity for an experienced PMHNP or new graduate interested in geriatric and long-term-care psychiatry. Role Your primary role will be providing psychiatric evaluations, medication management, and ongoing behavioral health care to residents across 4 nursing homes and 4 skilled nursing facilities . All facilities are located within approximately 5 miles of one another , making travel between locations convenient. There is flexibility in which facility you visit , allowing for a manageable and adaptable schedule. Key Responsibilities Perform psychiatric evaluations and follow-up visits Diagnose and treat psychiatric conditions Provide medication management and prescribe medications Monitor treatment response and medication side effects Manage behavioral and psychological symptoms in geriatric patients Develop and update individualized treatment plans Review medications, medical history, labs, and clinical information Complete timely clinical documentation Requirements Active Illinois PMHNP license Illinois controlled-substance license DEA registration available PMHNP board certification Strong clinical judgment and communication skills Interest in working with the geriatric population New graduate PMHNPs are welcome to apply! Important Start-Date Information This position is expected to begin approximately 6-8 months from the date of hire. We are hiring in advance of the clinical start date so that credentialing begins immediately upon hire . This allows sufficient time to complete credentialing and other required processes before you begin seeing patients.Benefits
