Family Nurse Practitioner / Physician Assistant

Job Description

Overview Join our dynamic healthcare team of Physicians and Nurse Practitioner, where you will play a vital role in delivering comprehensive inpatient, Outpatient and skilled nursing facility care across diverse medical settings. This energetic and rewarding position offers the opportunity to collaborate with multidisciplinary teams, utilize advanced clinical skills, and positively impact patient outcomes. You will be at the forefront of acute and chronic disease management, providing expert assessments, treatments, and patient education in a fast-paced environment. Your expertise will help ensure seamless care transitions, optimize patient recovery, and uphold the highest standards of medical excellence. Responsibilities Conduct thorough physical examinations and patient assessments to determine diagnosis and treatment plans in the inpatient settings, follows them in Acute rehab/ transitional care unit and in the outpatient clinic. Conduct Annual wellness visit, transitional care visits, and follow ups. Collaborate with physicians, and other consultants to coordinate comprehensive patient care. Utilize Athena to document medical records accurately and efficiently. Ensure compliance with HIPAA regulations and while maintaining high-quality patient care standards. Experience Proven experience working in inpatient and outpatient settings is required. Demonstrate proficiency with EMR/EHR systems such as Epic and Athena. Spanish speaking preferred
Pay:
$125,000.00 - $145,000.00 per year
