Job Description

Responsibilities:
  • Make rounds in Skilled Nursing Facilities to provide primary care to residents.
  • Provide comprehensive primary care services to geriatrics patients, including dementia care.
  • Perform physical examinations, diagnose and treat common acute illnesses and injuries.
  • Develop and implement treatment plans for chronic conditions and manage ongoing care.
  • Order and interpret diagnostic tests, such as medical imaging and laboratory tests.
  • Prescribe medications, including psychopharmacology, and provide patient education on medication use.
  • Collaborate with other healthcare professionals to coordinate patient care, including discharge planning.
  • Maintain accurate and up-to-date medical records using eClinicalWorks or similar electronic health record system.
  • Communicate with office staff to address faxes and telephone call messages from facilties.
Experience:
  • Must have a valid Nurse Practitioner license in the state of Illinois.
  • Strong knowledge and experience in geriatrics, dementia care, and other primary care services preferred.
  • Proficiency in using eClinicalWorks or similar electronic health record system.
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members.
  • Ability to work independently and collaboratively in a fast-paced environment.
