The Nurse Practitioner provides comprehensive evaluation, diagnosis, and ongoing management for individuals with chronic, complex, and preventive-health needs. You will order and interpret conventional and functional diagnostics, build individualized care plans, prescribe within scope, and coach patients on nutrition, sleep, stress, movement, and targeted therapies. Clinical care Conduct comprehensive intakes and follow-ups (typically 45-60 minutes for new visits; shorter for follow-ups). Perform history, physical exam, and functional assessment (systems review, timeline, lifestyle, environment, and psychosocial factors). Order, review, and interpret labs and diagnostics (standard panels plus specialty testing as appropriate: metabolic, hormone, thyroid, GI, nutrient, inflammatory, autoimmune, toxin/exposure-related, imaging). Diagnose and manage acute and chronic conditions within NP scope and collaborative practice agreements. Develop and adjust personalized plans that may include lifestyle interventions, nutrition, supplements, medications, hormone optimization, and referrals. Prescribe medications, hormones, and other treatments in accordance with state law, DEA authority, and practice protocols. Identify urgent or unexpected findings and communicate them promptly; coordinate higher-level or specialty care when needed. Educate patients in clear language; document shared decision-making and follow-up. Collaboration and operations Work with physicians, RNs, medical assistants, nutritionists, and front-office staff. Document same-day in the EHR using practice templates; maintain accurate, timely, HIPAA-compliant records. Participate in case review, protocol development, and quality improvement. Stay current on functional/integrative topics relevant to the practice (hormones, cardiometabolic health, gut, autoimmunity, longevity). Maintain licensure, DEA, BLS/ACLS (as required), malpractice coverage, and required CE/CME. Optional / practice-specific duties (include only if true for you) IV therapy, injections, or in-office procedures. Telehealth visits and asynchronous chart/lab review. Community education, workshops, or light content support. Qualifications Required Master's or doctoral degree in nursing (MSN or DNP) from an accredited program. Current, unrestricted NP license in [IL]. National board certification (AANP or ANCC) as FNP, AGNP, or equivalent adult-focused NP. [2-3]+ years of NP clinical experience (primary care, internal medicine, women's health, or outpatient preferred). Strong assessment, lab interpretation, documentation, and patient-communication skills. Comfort with EHR and telehealth platforms. Commitment to root-cause, patient-centered care and evidence-informed functional practice. Preferred Prior functional, integrative, or lifestyle-medicine experience. Training or certification through IFM, A4M, or equivalent (or willingness to complete structured training). Experience with advanced labs (e.g., comprehensive metabolic/hormone panels, GI-MAP, DUTCH, organic acids) and translating results into plans. Experience with HRT/BHRT, metabolic/weight management, gut health, or autoimmunity. Multi-state or compact licensure if the role includes telehealth across states. What success looks like Patients leave visits understanding their results and next steps. Plans are individualized, documented, and adjusted based on follow-up data. Collaboration with the team is timely and professional. Care stays within scope, protocols, and regulatory standards.
