$135,000-$155,000 per year, based on experience NG Vascular & Vein Center is seeking a compassionate, motivated, and highly organized Nurse Practitioner to join our growing mobile clinical team. This is a unique opportunity to bring specialized vascular and wound-care services directly to patients in skilled nursing facilities, rehabilitation centers, and long-term-care communities throughout Chicagoland and the southwest Chicago area. Instead of working in one clinic every day, you will serve as a trusted clinical partner to nursing-home residents, families, facility staff, and referring providers. You will evaluate patients with vascular disease, chronic wounds, leg pain, swelling, and circulation concerns while helping high-risk patients receive timely specialty care. What You Will Do Travel to assigned skilled nursing facilities, rehabilitation centers, and long-term-care communities. Perform comprehensive consultations and follow-up evaluations. Evaluate patients for peripheral arterial disease, chronic limb-threatening ischemia, venous insufficiency, leg swelling, chronic wounds, dialysis-access concerns, and related vascular conditions. Obtain focused histories and perform vascular, wound, and general physical examinations. Review available laboratory results, vascular studies, imaging, medications, and prior medical records. Identify patients who may benefit from arterial or venous testing, wound-care services, vascular intervention, or physician evaluation. Develop evidence-based assessment and treatment plans in collaboration with NG Vascular physicians. Write clear, accurate, and timely consultation and follow-up notes in the electronic medical record. Place appropriate orders and coordinate vascular testing, follow-up appointments, and referrals. Communicate recommendations with nursing-home staff, primary-care providers, patients, families, and the NG Vascular clinical team. Monitor patients after vascular procedures and help ensure continuity of care. Educate patients and facility staff about vascular disease, wound prevention, compression therapy, risk-factor modification, and signs requiring urgent evaluation. Maintain accurate scheduling, facility communication, and documentation of completed visits. Follow HIPAA, Medicare, facility, and NG Vascular documentation and compliance standards. What We Are Looking For Current Illinois Advanced Practice Registered Nurse license. Current national certification as a Nurse Practitioner. Family, adult-gerontology, primary-care, or acute-care certification. Current BLS certification. Valid driver's license, reliable transportation, and willingness to travel throughout the assigned territory. Strong clinical assessment, communication, and documentation skills. Ability to work independently while collaborating closely with physicians and facility-based clinical teams. Comfortable using an electronic medical record and completing documentation promptly. Professional, dependable, compassionate, and highly organized. Preferred Qualifications Experience in skilled nursing, long-term care, geriatrics, vascular medicine, wound care, cardiology, podiatry, or internal medicine. Experience evaluating chronic wounds, peripheral arterial disease, venous insufficiency, edema, or diabetic foot complications. Wound-care certification or interest in obtaining certification. Familiarity with Medicare documentation and medical-necessity requirements. Bilingual English/Spanish skills are a plus.
