This role offers a flexible schedule (Monday-Friday, no nights or weekends), with opportunities for both part-time and full-time employment (W2). Responsibilities include performing physical assessments, diagnosing and managing infectious diseases, prescribing medications, developing care plans, coordinating referrals, and ensuring accurate documentation. The role involves building relationships with residents, families, and care teams to improve outcomes and reduce re-hospitalizations, with a minimum of 12 patient visits daily. Qualifications include an active Illinois NP license, a Master's degree in Nursing, and infectious disease or long-term care experience preferred. Strong communication skills and knowledge of geriatric care are advantageous. Compensation starts at $50/hour plus monthly performance bonuses.
