Nurse Practitioner - Diabetes Care Flexible Schedule Supportive Team Rewarding Specialty Work About the Role Dialog Healthcare LLC is seeking a dedicated Nurse Practitioner (NP) to join our growing specialty diabetes program. You'll provide ongoing endocrine and diabetes management to residents in a large, well-maintained long-term and sub-acute facilities in need of expert, compassionate care. You'll conduct on-site and potentially telehealth visits, monitor and adjust diabetes treatment plans, and collaborate with facility staff to ensure optimal blood glucose control, prevention of complications, and improved patient outcomes. This position is ideal for a Nurse Practitioner who enjoys autonomy, values continuity of care, and wants to make a measurable impact on patients' lives. You'll work closely with our Clinical Director and receive hands-on support and training in evidence-based endocrine care—gaining advanced clinical skills that are both marketable and meaningful. Position Details Work Setting:
