Requirements:

Certified Nurse Practitioner with 2+ years in Skilled Nursing preferred, excellent communication and organizational skills, ability to travel within 12-20 miles, and thrive in a fast-paced environment.

Responsibilities:

Collaborate with patients and multidisciplinary teams to develop treatment plans, conduct assessments, diagnoses, and treatments, create weekly rounding schedules, and maintain accurate records.

Work Environment:

Day shift, flexible hours (8-40 hours/week), in-person work, with a focus on providing evidence-based care in a collaborative setting.
