is looking for a compassionate and driven House Call Nurse Practitioner to join their team in Warrenville, IL. Responsibilities:

Conduct comprehensive in-home patient assessments, evaluations, and follow-up visits Diagnose and manage acute and chronic conditions for homebound patients Develop and implement individualized care plans in collaboration with physicians and care teams Prescribe medications and order diagnostic tests as appropriate within scope of practice Coordinate with specialists, home health agencies, and other healthcare providers to ensure continuity of care Maintain accurate and timely clinical documentation in the EMR system Educate patients and families on disease management, medications, and preventive care

Requirements:

Active Nurse Practitioner license in the state of Illinois Master's degree or higher in Nursing (MSN, DNP) with NP certification (FNP, GNP, or AGNP preferred) Experience in primary care, geriatrics, or home-based care strongly preferred Valid DEA license and ability to prescribe medications Reliable transportation and valid driver's license for travel to patient homes Strong clinical assessment and critical thinking skills in independent settings Excellent communication, empathy, and patient-centered bedside manner
