Job Description

Indiana University Health Medical Group-Metro Region is seeking a Nurse Practitioner or Physician Assistant to join our ACE (Acute Care of Elders) team at IU Health West Hospital in Avon, IN! ACE is an inpatient consults program at IU Health Methodist Hospital, IU Health North Hospital, and IU Health West Hospital. We supplement inpatient care with an emphasis on key geriatric syndromes, falls, and decline in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Position Details:
Full-time, Day Shift M-F, 8:00am-4:30pm No weekends or call, allowing for work/life balance 100% inpatient Expected patient volume in the hospital 10-15/week
Team:
1 physician and 2 advanced practice providers Once onboarding and inpatient practice is established, outpatient session(s) will be expected to facilitate continuum of care for patient post-hospitalization
Qualifications:
