About the Role As a Nurse Practitioner, you will play a critical role in delivering comprehensive healthcare services to a diverse resident population. You will be responsible for assessing resident health, diagnosing conditions, and developing individualized treatment plans to promote optimal health outcomes. This position requires collaboration with interdisciplinary teams to ensure continuity of care and effective management of chronic and acute illnesses. You will also provide resident education and support to empower individuals in managing their health. Ultimately, your expertise will contribute to enhancing the quality of life for residents while upholding the highest standards of clinical practice and resident safety. Job Responsibilities Conduct thorough resident assessments including taking vital signs and reviewing medical histories. Diagnose and treat acute and chronic illnesses, injuries, and other health conditions. Develop and implement individualized care plans, including wound care and infection control protocols. Collaborate with physicians, nurses, and other healthcare professionals to coordinate resident care. Document resident information accurately using electronic health records and maintain compliance with healthcare regulations. Provide resident education on disease prevention, medication management, and healthy lifestyle choices. Monitor resident progress and adjust treatment plans as necessary to ensure effective outcomes. Minimum Qualifications Master's degree in nursing (MSN) or higher from an accredited program. Current Nurse Practitioner (NP) licensure and certification in the state of Indiana. Basic Life Support (BLS) certification. Demonstrated experience in resident care, including vital signs monitoring and wound care. Proficiency in basic computer literacy, including electronic health record (EHR) systems. Preferred Qualifications Certification in Infection Control or related specialty. One year Experience working in long-term care or geriatric healthcare settings. Advanced certifications such as Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP). Familiarity with quality improvement initiatives and healthcare compliance standards. Strong communication and leadership skills to support interdisciplinary team collaboration.
