Assess, diagnose, and manage patients across all ages, emphasizing prevention and health education. Collaborate with a multidisciplinary team to coordinate holistic care. Maintain accurate documentation in the EHR and participate in quality improvement and population health initiatives. Support health promotion efforts targeting underserved communities.
