Dragonfly Primary Care is a small, private practice dedicated to providing high-quality, personalized healthcare to the Franklin Township community in Indianapolis. Our practice is rooted in compassion, collaboration, and a patient-centered approach. Position Overview:
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team. The ideal candidate will have experience in primary care, be committed to providing patient-focused care, and be capable of working independently as well as part of a collaborative team. This role involves delivering comprehensive care to patients of all ages, managing chronic conditions, and promoting preventive health measures.
Key Responsibilities:
Conduct thorough physical exams, assess patient health, and diagnose and manage acute and chronic conditions. Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team. Prescribe medications, treatments, and interventions as appropriate. Provide patient education on health promotion, disease prevention, and management of chronic conditions. Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care. Maintain accurate and up-to-date patient records using our electronic health record (EHR) system. Participate in quality improvement initiatives and contribute to the continuous improvement of practice operations.
Qualifications:
Master's degree in Nursing (MSN) from an accredited program. Current Nurse Practitioner (NP) license in the state of Indiana. Board certification as a Family Nurse Practitioner (FNP). Minimum of 3 years of experience as a Nurse Practitioner. Strong clinical skills and ability to make sound decisions in patient care. Excellent communication, interpersonal, and organizational skills. Proficiency in using electronic health records (EHR) systems.
