Job Description

Provide patient-centered primary care to residents in skilled nursing facilities across Mishawaka, Indiana, as part of a growing clinical team. This part-time role offers flexible scheduling, six-hour facility shifts (rounds typically start between 7:00-10:00 a.m.), no call or overnight responsibilities, and collaborative work with physicians and facility staff to deliver evidence-based care. Locations also available nationwide in GA, IL, KS, MI, MO, OH, TN, and WI. Key responsibilities Perform daily rounds and manage acute and chronic conditions for 15-20 patients per day on average Prescribe and adjust medications, develop and implement treatment plans Communicate and coordinate care with nursing staff and interdisciplinary teams Maintain clinical documentation and ensure compliance with regulatory standards Compensation & benefits Competitive pay with earning potential above industry benchmarks Long-term retention incentive up to $50,000 and performance-based bonuses Paid time off, 401(k) with employer match Medical, dental, and vision coverage for eligible providers Employer-paid life insurance; short- and long-term disability CME allowance and licensure reimbursement Company-provided iPad with LTE and professional lab coat Qualifications Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license NP board certification (ANCC, AANPCB, PNCB, or AACN) or PA certification (NCCPA) Board certified or eligible in FM, IM, Geriatrics, PM&R, or Emergency Medicine Strong clinical judgment, communication skills, and ability to work independently while collaborating within a multidisciplinary team
