Full Job Description Position Summary:
As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will provide comprehensive primary care services within skilled nursing and senior living communities. This role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Qualifications:
Current NP or PA licensure in the state of practice Previous professional patient care experience Experience in geriatrics, family medicine, or internal medicine preferred.
