JOB SUMMARY AND SPECIFICATIONS JOB SUMMARY

Collaborates and consults with Medical Director to provide comprehensive health care to patients through an organized, established program that addresses the specific needs of the patient. Examines current practices to improve patient care outcomes. Collaborates with entire healthcare team to address complex patient problems, developing systems and practices that provide for continuity and communication between team.

MINIMUM QUALIFICATIONS

Professional & Technical Skills Demonstrates leadership, management, and communication skills. Working knowledge of department operations. Comprehensive knowledge and skills in current nursing practice, wellness, and disease management principles. Education Master's Degree in Nursing or Physician Assistant License(s) or Certification(s) Current Indiana License to practice as a Nurse Practitioner, CPR & ACLS certifications Skilled to Care for Certain Age-Related Patient Groups (incumbents will be skilled in the care of the following patient groups) _____ Does not apply _____ Adolescence (13-17 years) __X__All age groups _____ Infants (0-1 year) _____ Geriatric (65+ years) _____ Pediatric/Early Childhood (1-12 years) _____ Adult (18-64 years) Other Skills or Requirements As per credentialing file.
